A medical report card was evidence in the trial of Ryunosuke Naruhodo for the murder of John Wilson. According to the data in the report, Wilson had undergone a tooth extraction not long before the murder and was instructed to consume only water for the next three hours.

Wilson arranged a lunch meeting with one of his students, Jezaille Brett, at the restaurant La Carneval on November 22. Due to an appointment for a tooth extraction at 1 p.m. that day, the lunch was set to 2 p.m., and only Brett would order food. Unbeknownst to Wilson, Brett would use this opportunity to assassinate him without leaving a trace, using curare. This toxin was unknown to investigators in Japan at the time, and was the subject of one of her research reports.

The table was served with one plate of steak and a bottle of carbonated water. Brett laced the water with the poison; although both of them drank from it, the poison would only work if it entered through a wound, so only Wilson, who had undergone a tooth extraction, succumbed. A photographic print of the crime scene taken after the fact showed the table with Brett's steak plate, giving the impression that Wilson had dined alone. The medical report card from Wilson's appointment was retrieved from his pocket and submitted into evidence.

During Naruhodo's trial, Iyesa Nosa and Kyurio Korekuta were called as witnesses. They testified that Wilson had been dining by himself at the time of the crime, making Naruhodo, who had approached Wilson shortly before the murder, the only possible culprit. Prosecutor Taketsuchi Auchi corroborated their testimony with the crime scene photograph. Naruhodo claimed that Wilson had actually been dining with a woman, and cursed the fact that there had been so few witnesses. This prompted Judge Seishiro Jigoku to ask why Wilson had visited the restaurant at 2 p.m., to which Auchi submitted the medical report card. Naruhodo examined the details of Wilson's appointment and concluded that he could not have been the one dining on the steak plate, and that the plate was for someone else.

Brett was eventually called to the stand, and Naruhodo accused her of poisoning the water. Although no poison was detected on the bottle, Susato Mikotoba managed to retrieve Brett's research report on curare. In an attempt to "disprove" Naruhodo's accusation, she drank the remaining water in the bottle, knowing that it would not harm her. However, Mikotoba's translation notes on Brett's report revealed that the poison would have only affected Wilson, as only he had an open wound in his mouth from the tooth extraction procedure.
